Home Visits

To gain a full understanding of each patient’s circumstances and lifestyle, our comprehensive senior care incorporates the home visit into our practice. Home visits complete an evaluation allowing for the development of a treatment plan tailored to patient goals.


In home we will observe and make recommendations regarding:

  • physical and environmental conditions and their effect on daily life
  • emotional and cognitive function and their impact on well-being
  • home safety
  • nutrition access
  • mobility and self care
  • support network and the need for services, including: visiting nurse/homemaker services; physical, occupational, and speech therapy; nutritionist education and guidance; social services and recreational supports; supportive equipment and/or living space redesign

Ambulatory Care and Office Visits

We have developed a modern, comfortable office for patients and their families. The office has full handicap accessibility. Visits will be scheduled at the office for 45 minutes to 1 hour, to allow for a thoughtful evaluation. Office visits may include:

  • preventative care
  • annual wellness exams
  • sick visits (if patient prefers to be seen outside of the home)
  • routine/follow up exams
  • minor surgery
  • testing
  • education
  • counseling
