In home we will observe and make recommendations regarding:
- physical and environmental conditions and their effect on daily life
- emotional and cognitive function and their impact on well-being
- home safety
- nutrition access
- mobility and self care
- support network and the need for services, including: visiting nurse/homemaker services; physical, occupational, and speech therapy; nutritionist education and guidance; social services and recreational supports; supportive equipment and/or living space redesign

Ambulatory Care and Office Visits
We have developed a modern, comfortable office for patients and their families. The office has full handicap accessibility. Visits will be scheduled at the office for 45 minutes to 1 hour, to allow for a thoughtful evaluation. Office visits may include:
- preventative care
- annual wellness exams
- sick visits (if patient prefers to be seen outside of the home)
- routine/follow up exams
- minor surgery
- testing
- education
- counseling
